(i) Handling:
Records indicate in June 2013, a juvenile baboon died after being caught in the cable for a guillotine door. A cable
that was accessible to the animal, as wall as incorrectly anchored allowed the animal ta reach the cable. The animal
slipped the cable around his neck resulting in strangulation. In.a 12 month period, two juvenile macaques died from
similar strangulation incidents involving guillatine door cables in another housing area at the facility. In response ta
the first incigant, caretakers working in thal housing area were retrained in the proper way to secure the guillotine
doors and modifications were made: to the door cables so they were nat accessible fo the animals, However, the most
recent death accurrad in a housing area where the cable doars had not bean modified and ware stil aooassible to the
animals. Cables for all guillotine doors must not be accessible ta the animals and caretakers must anchor the door
handles properly to prevent physical harm when handling the animals,

Atabie in @ hallway outside of 2 animal rooms was cluttered. There was.an open bag of cereal and coral bars next to
Gleaners and disinfectants . Tools were scattered on the table . Open bags-af food must be properly stoced in
Containers to prevent contamination and all toxic items such as cleaners stored away fram food and food prep areas
to maintain husbandry peactices and protect the animals trom injury.
