Two juvenile macaques have died in the last 3 months as a result of becoming éntangled in the ¢able tor the
guillotine door between the indoor and outdoor sections of tha enclosures. Cable handles were unsecured on each
incident allowing the cable to be accessed by the animals. The deaths resulted from improper securing of the
Quillatine door, All personne! must properly secure doors Lo pravent-escape, injury or death, Training documents wore
reviewed that showed that all personne! have now been trained in the proper maintenance of guillotine doors and
animal shifting practices. CORRECTED BY TIME OF INSPECTION

PRIMARY ENCLOSURES,
Primary enclosures for nonhuman primates must meet the folowing minimum requirements:
(a) General requirements, (1) Primary enclosures must be designed and constructed of suitable materials so that
they are structurally sound for the species of nonhuman primates contained in them. They must be kept in good
repair (2) Primary enclosures must be consiructed and maintained so that they:
(i) Have ne sharp points oF edges that ould injure the nonhuman primates
‘The polycarbonate panels in 2 enlcosures have several cracks and are broken creating sharp edges which may
cause injury to the animals, One chain supporting a hanging barrel is exposed and thera is a sharp piece of metal
